Prepare your notes for your next visit

Turn day-to-day observations into a clear set of dates, details, and questions to discuss with your clinician.

Describe what you noticed

It can be hard to recall an ordinary Tuesday several weeks later. When adding a symptom observation, use words you would use in conversation: what you felt, when you noticed it, and how it affected your day. You do not need a medical term or an explanation. A description of your experience gives you something concrete to return to.

Keep timing in the picture

Include the date and any timing you remember, such as the part of the day or roughly how long the experience lasted. If it happened again, record that observation separately. You can review these entries alongside your injection history, food, and water records. Events appearing near each other on a timeline are details to discuss, without assuming one caused another.

Choose what you want to discuss

Before your visit, open myMON and review the period since your last conversation. Pick the observations you most want to mention and write a short list of questions in your own words. Keep the relevant dates next to them. This gives the conversation a starting point without requiring you to tell the whole story from memory.

Let your notes support the conversation

During the visit, open the records that relate to each question. Say when an entry reflects a clear memory and when a detail is uncertain. Leave room for your clinician to ask follow-up questions and interpret the information with you. Your notes do not have to be polished; they only need to help you explain your experience more clearly.
